Yep, the Ahesive cover on the back of my palm TX was pealing so I decided to replace it. :D
Here's a simple template I made:

Just insert your information (preferably in white) and place a scan of your S/N in the lower part.
I printed it out with my photoprinter (took a few tries to get the size right) and used regular clear packing tape to tape it over the pealing one. :D
