Hey folks,
I'm a family medicine resident at a high-volume county hospital system. I think i've got a system down with tracking my patients in the hospital, and it mostly uses pen and paper. But for my patients in the (paper not electronic records) clinic, there's so much i want to keep track of:

- names and clinic number of the patients i've seen
- something personal about the patient to remember them by in future encounters
- past medical history
- current meds
- labs and studies to follow-up on (so i can come back to this patient, see that i referred for a mammogram or a colonoscopy, and see if the paperwork went through and it was done or if it's something i have to pursue again)

in my county residency system, patients have a lot of chronic co-morbidities and there are a ton of things to follow up on. and our clinic charts don't always have the latest note, or the things i wanted to follow up on when i saw the patient 2 months ago.

with a patient tracking software system i'd have the added time-consuming piece of having to enter in all the patient's info into my computer and sync it with the patient tracking system, but it would be so worthwhile, especially as I start to have more continuity with my patients. i wouldn't be pulling my hairs out with trying to piece together the pmhx or meds or things to followup on.

my question -- what, if any, software do you recommend for patient tracking? freeware or for purchase? pluses, minuses? what's better for outpatient, etc? i'd love your thoughts on this. thanks.

Hello Anjali.

I have not seen many medical professionals acknowledge themselves as such here at 1src. There is a Palm Medical forum at Brighthand. You may have better success locating information there. There is also a blog called the Palmdoc Chronicles. Good luck.
